The Care Coordinator performs comprehensive needs assessments related to community-based care with older adults. The position assists clients, families, caregivers, and grandparents with short-term services that help older adults age in place and support individuals with their family caregiving efforts through innovative person-centered case management and care planning. AAACAP promotes independence and plans for a quality aging and disability support network that is community-based, and family centered. Education: Graduation from high school or an equivalent required. Bachelor’s degree in a human services field is preferred.
Experience: Two years of experience in human service programs is required.
Other Requirements: Applicants must maintain a valid Texas driver’s license, proof of insurance specified by the State of Texas, and dependable transportation as travel in the CAPCOG 1o-county region is required.
Salary Range: $45,000 – $49,000
