PRIMARY RESPONSIBILITIES

To manage the Family Caregiver Support and the Evidenced Based Integration (EBI) programs in the Area Agency on Aging (AAA), including development of program policies, creation of program monitoring tools, oversight of all grants, compliance with state requirements; Identification of existing resources and gaps, and outreach activities related to the development of the program

 

TASKS PERFORMED

This is a responsible supervisory position requiring familiarity with various government programs related to the elderly, as well as an ability to create and provide administrative oversight to the caregiver and EBI programs. Duties include developing, implementing, and coordinating the policies
and services for the caregiver and EBI programs throughout AACOG region; Conducting needs assessments and developing the appropriate plan to meet those needs; Monitoring of budget; Analyzing legislation related to Caregiving and EBI issues; Coordinating caregiver and EBI activities
within AACOG region; Evaluating the effectiveness and quality of the programs; Analyzing existing resources to meet caregiver needs and demand for EBI programs; Identifying gaps in service, and develops plans to meet those needs; Develops program structure and the policies and procedures for AACOG region

 

ESSENTIAL FUNCTIONS

Monitors the program budget and processes billings; Oversees all program grant awards and activities; Prepares caregiver and EBI program portion of Health and Human Service Commission (HHSC) reports, including quarterly reports, client demographic reports, closeout reports, area plan tracking, assurances, and budgets; Researches and analyzes ordinances, legislation concerning caregiver health and wellness issues, and reviews Federal and Texas registers and other periodicals for information relevant to the program; Organizes and coordinates the Alamo Caregiver Support Coalition, a Committee of the AAA Senior Advisory Committee; Develops training materials for caregiver support groups, community groups, and other agencies, and conducts training as necessary; Develops outreach materials and conducts outreach activities, including speaking to community organizations and the media about caregiver support and health and wellness issues; Develops caregiver support groups, including facilitating meetings with the goal of self-sufficiency, monitoring the status of caregiver support groups, and providing assistance as needed; Recruits, supervises, and trains volunteers to assist within caregiver support and health and wellness activities; Develops collaboration within AAA region, including identifying resources, determining site selection, and fostering agreements with community partners; Manages the development and implementation of current and new evidence based disease prevention and health prevention programs for the agency; Performs other job related duties as may be required

DESIRED JOB EXPERIENCE

Two (2) years of progressively responsible experience in the field of aging, social work, other human services, or a related field. (Two years of full-time aging, social work, or other human services experience may be substituted for one year of the required education, with the maximum substitution of four years of experience for two years of education) OR any equivalent combination of experience and training which provides the required knowledge, skill, and abilities

 

OTHER QUALIFICATIONS

Must become a lay leader within three months or within three months of implementation of a new evidence-based program by the agency (based on availability of funding)
