Capital Area Council of Governments, CAPCOG, is a regional organization of city and county governments in the 10-county region surrounding the Austin metro area created to foster collaboration at the regional level and promote economy and efficiency in the coordinated development of the region. Area Agency on Aging of the Capital Area (AAACAP), a division within CAPCOG, that provides quality services to support the health, safety, and well-being of older individuals in the 10-county Capital Area through a variety of programs and activities.

With its wide range of innovative projects and programs the Area Agency on Aging offers employees opportunities for participation in local, state and federal projects on the leading edge of community-based, family centered, service provision. Agency operations promote independence and plan for a quality aging and disability support network in response to the rapidly expanding regional population of older adults and their caregivers.

The AACAP is seeking candidates to be an active part of their team as a Care Coordinator/Intake Specialist. AAACAP is passionate about our mission and values the individuals and families we serve, ensuring they receive the highest quality of customer service. AAACAP seeks an adaptable team player who can gather information and has experience working with numbers, policies and diverse populations to determine eligibility for social service programs in a fast-paced environment. Work involves developing and maintaining contact with clients, client families, caregivers and service providers to meet service needs. Work requires application of specialized program knowledge and service skills used to conduct initial screenings, complete assessments, develop care plans, authorize appropriate services, and process monthly billing. Work includes planning, outreach, program support and providing technical assistance.

Travel is required within the 10-county region. The incumbent must maintain a valid Texas driver’s license, proof of insurance specified by the State of Texas, and dependable transportation.

Education:

Graduation from an accredited four-year college or university with major course work in social work or a related field is preferred.

Experience:

At least two years of experience in social service programs is required.

Bi-lingual Spanish preferred

Experience and education may be substituted for one another.

Salary range: $20.08 per hour
