This is a responsible supervisory position which Performs complex case management work. Work involves serving as a Liaison between clients and their families in obtaining assistance and rehabilitative treatment. Delivers a managed care system that ensures appropriate access, utilization, and effectiveness. Works with considerable latitude for the use of initiative and independent judgement. Serves as point of contact with veterans, providers, their dependents and families to provide connections for various services. Essential Functions include: Knowledge of veteran community, community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services; Requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques; Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA; Perform other job related duties as assigned. Bachelor Degree preferred and equivalent experience. Experience in veteran’s programs and social services work. Graduation from an accredited four-year college or university with major coursework in behavioral sciences, social sciences, humanities, or a related field is generally preferred. Experience and education may be substituted for one another. Knowledge of mental illness and emotional components of physical illness; of rehabilitation methods; of casework principles; of community resources; and of the principles, methods, techniques, and practices of social work. Skill in interviewing. In the absence of the Coordinator, have the ability to oversee the program, evaluate reports and assessments, communicate effectively, and to supervise the work of others. Bilingual English/Spanish preferred. AACOG business hours 8:00 a.m. – 5:00 p.m. M-F. This position is a Pay Grade 10, with a starting hourly rate of $17.69. This is a non-exempt position.
